For people whose lives have been profoundly affected by aging, accidents or illnesses, our center offers the opportunity to live a life as comfortable and fulfilling as possible. Not only do we offer expert medical, nursing and therapeutic services, we also provide a warm, comfortable and caring environment for long-term living.
Long-term care at Hamilton Plaza Nursing and Rehabilitation Center is designed to meet the physical, emotional, intellectual, social and spiritual needs of our residents. A 24-hour care team is always on call for routine and emergency medical needs, while our excellent nursing team works to keep residents as flexible, mobile and independent as possible. Our activities department coordinates a full program of activities and events to address residents' cognitive, emotional and spiritual needs, with input from medical professionals, gerontologists, psychologists, and members of the clergy.
Long-term care often means that some individuals will spend months or years with us. Our center becomes their home: we become their second family. That kind of relationship is a privilege and a trust, and we honor it by treating each resident with kindness and courtesy, mindful of their dignity as human beings and of our responsibility for their health and happiness.
"I came here because I couldn't live alone anymore. My heart wasn't strong enough. But I still have my sense of humor and I still enjoy making friends and staying active. They will keep you busy here! There are plenty of activities."

"There's always something fun and interesting happening here! We have summer barbecues, visiting musicians, and tons of parties."
